We believe that knowledge is power.

Through our Diabetes Education programs, we provide patients with all of the education, resources, and products to help them stay healthy. Because the more you know, the better you will be able to manage your diabetes.

Our team includes registered nurses and registered dietitians who are certified diabetes educators.

Our Program

Now conveniently located within several of our primary care offices, access to diabetes education is simple and easy. The Diabetes Education program is recognized by the American Diabetes Association — the gold standard in meeting the requirements of diabetes self-management education.

One-on-One Diabetes Instruction

Our One-on-One Diabetes education programs offer patients individual consultations to help find the best diabetes management plan for them. Individual counseling sessions may cover:

  • Type 1 diabetes
  • Beginning insulin
  • Intensive insulin training
  • Continuous glucose monitoring (CGM)
  • Prediabetes teaching
  • Insulin pump management
  • Specific follow-up visits

Nutrition Counseling/Medical Nutrition Counseling (MNT)

A one-on-one counseling session with a registered dietitian may be scheduled to discuss a variety of diagnoses or healthy eating needs.
